Disaster impact survey for primary producers

The disaster impact survey for primary producers is a voluntary and anonymous survey for primary producers to tell us about the impacts of a natural disaster event.

Purpose

When you complete this survey, you’re helping us to assess:

  • how disasters have affected your property
  • whether disaster recovery assistance needs to be activated.

It is not an application for assistance.

Your survey response can be used as supporting evidence for your application once support packages are announced.

Timing

Complete the survey soon after a natural disaster to help us to activate disaster recovery assistance as quickly as possible.

How to complete the survey

  1. Open the survey and complete all fields to the best of your knowledge—we understand that the full impacts may not be known for some time.
  2. If you are reporting multiple properties, complete a separate survey for each one.
  3. If you wish to receive a copy of your submission or update it later, please provide your email address.
  4. Submit the survey.

How the information is used

Your information may be shared with other Queensland state and local government agencies responsible for disaster response and recovery. We will only use your information for this purpose.

It will otherwise not be used or disclosed unless authorised or required by law. Any personal information will be handled in accordance with the Information Privacy Act 2009.

After submission

After you submit your survey, and if you provided your email address, you will receive a link to enable you to update the survey if needed.
